A great application for creating both surgical preference cards and also surgical instrument count sheets is ISIP Manager. The application is free for individuals and there are paid accounts for larger surgical centers and hospitals.
We created an account to see how ISIP works along with a user account so you can have a look with our password below without creating your own account. The user account only allows you to view the information, you have to create your own account if you want to edit or create preference cards. But since it's free for a basic account, there's no reason not to try it out.

We signed up for the application by providing our email address and other details, but no credit card was necessary. The account already has a variety of surgical instruments and preference cards pre loaded in the account to get you started. And there is a user guide to download from the main web page. All in all, a very nice application to create and update surgical preference cards and instrument sets.
